Final Farewell From Dr. McCormick:

It has been a wonderful journey over these past 43 years, and now it is time for me to move on in some new directions. I have come to know your children almost more than I know my own grandchildren, so it is time to spend more time with them in San Diego, time to play a bit more tennis, and time to take up some art projects that have been tickling the back of my mind for years. As much as I have enjoyed participating with you in the raising of your children, I realize that I no longer have the stamina to keep up the pace, particularly with the numerous changes in insurances and governmental regulations that have added an enormous additional burden to the practice of medicine. I'm afraid that I am simply a dinosaur with my old-fashioned solo practice and real people answering the phone when you call us, and as with the dinosaurs of the past, it is time to evolve into other kinds of things. I wish you and your children all the best in the futures ahead of you.

Specify you own name, your relationship to the patient, and the names and birthdates of your children. Please let us know if you want only immunization records and/or a record of all office contacts. We will reply to your email request with those records attached in the form of pdf documents. Please make sure to keep printed or digital copies of these records for future use. We suggest copying the pdf documents to a cd or to a thumb drive for safe keeping.
